• Directly: Information is collected via our intake forms, during consultations, and through email or phone.

• Secure Storage: We use Halaxy, a secure practice management system, to store clinical notes and sensitive data. Halaxy uses bank-grade encryption and stores data within Australia.

• Business Admin: We use Google Workspace for administrative communication. We have a Business Associate Agreement (BAA) and security protocols in place to manage this data.

4. Purpose of Collection

We collect your information primarily to:

• Provide personalized nutrition assessment and advice.

• Communicate with your GP or other health providers (with your consent).

• Process Medicare or insurance claims.

We will not share your information with third parties without your consent, unless:

• It is necessary to provide your health service (e.g., writing a report to your GP).

• We are required or authorized by law (e.g., subpoena).
